i need thew original copy so i can make edit and add-ons in the future

This is my website the final product will being in the "rates" section of it http://www.c-wconstruction.com/

The file attached below has all of the information you will need. What i list below is what i want changed.

i would like to change and combine the "nail wood flooring and glue wood flooring" into item that says "Hardwood Floor Installation" and the price should be " $2.50-$3.50 s/f)

I would also like to remove the section titled" Moisture barrier install"

And i would also like to add in "Tile Installation" and the price should be "$3.00- $4.50 s/f"

The information does not have to be in the same order as below, but i would like the design to be the same or very similar, i would also like the color scheme to match my website and logo.

I look forward to seeing the results.
