Design Brief

Program to use: Illustrator (compatible for latest version + CS5)
I will require the final PDF + the Illustrator file

What:
The document I require will be called the ‘Event AV request form’
This document will basically be an information-gathering document, which will allow us to receive information from a client in order to best quote on their event.

From the final PDF received, I will be transforming the document into an interactive PDF, and will be adding drop boxes and checkboxes.
What this means, is I will require space after the various headings.
I have inserted instructions after each heading with what I require

I have attached all the content that you will need to use. This document is called 'Audio Visual Event Request Form'

I have an example of what I mean by this and will be able to send it through. This example version is not what I’m after astatically, its just to give you a better idea. These documents are called 'example 1 and example 2'

Please use as close as possible typeface font as the words ‘lighting sound and vision’ on the provided logo

Please use the attached logo to the document (up the top of the document) The document name attached is 'Wired4Sound Logo Inverted' and is a .png file
Please use the logo, as a guide to the look and feel of the document so there is some continuity. I require a sleek / professional but not too boring document that is easy to read and presentable.

Whilst there is quite a lot of instructions, this will be quite an easy document to put together. Since there is quite a bit of text and space needed for checkboxes and text boxes, I expect this will probably need to go over 2-3 pages (but no more than 3 pages)
