design a A4 size cover page and a header and footer for a PDF file

Contest Brief

I need someone to design a ( A4 size ) cover page and a header and footer for a PDF file so I can put it in my files, the designs can be in illustrator or Word or even a Jpeg with the attached logo of the institute.

*the logo color is White and Green so make sure you are viewing the logo correctly

* I Need this ASAP so I might choose a winner from the first design that I like

*I have attached a picture of the owner maybe it could help you in the design of the cover page
