Need a logo for a new dental practice and a business card incorporating the logo.

The requirements for the logo are as follows:

- Name of the practice is "Top Health Dental"
- Logo graphic and the business name needs to be independent of each other
- Colour scheme of the logo can consist of 2-3 colors
- Logo graphic MUST NOT use a tooth
- Logo will be printed on perspex and displayed in a room with cream walls and green highlights
- the logo must be able to be easily reproduced and placed on a website, business card and signage.

Business card requirements are:

- Front side to incorporate business name, name of the dentist, logo, address, phone, fax, email and website address
- Reverse side is for the practice to write down the next appointment time and date (eg. "Your next appointment is on __________ (DD/MM/YY) at __:__ am/pm with Dr ______________)

File type requirements:
- Adobe Illustrator (.ai), editable PDF file, PNG file and JPG File
- transparent background
- CMYK colour mode
- Business card document size: 100 x 65mm (5mm bleed) (keep all important information within 87 x 52mm)
