Project context The problem Falls in UK care homes drive 255,000 emergency hospital admissions per year and cost the NHS £2.3 billion. The single biggest preventable contributor is staff acting on incorrect mobility information — assisting alone when two staff are needed, or hovering when a resident is independent. Agency and bank staff are particularly vulnerable to this gap because they don’t know individual residents. The clinical assessment of mobility risk already exists — the Multifactorial Falls Risk Screen (MFRS) is the established 13-factor falls assessment used across UK care homes. The problem is that MFRS data lives in the care plan, not in the moment of action. Nobody reads 13 risk factors before helping a resident from a chair to a commode. The solution A two-part system: 1. NFC tags placed where care happens — on mobility aids (where present), bed-frames, door-frames, and optionally on consent-driven silicone wristbands. Multiple tags per resident; all resolve to the same record. Body-attached / adhesive tags are explicitly out of scope. 2. An Android app for staff. Tap phone → instant display of traffic light status + 3–5 short clinical flags drawn from the resident’s MFRS assessment. Behind it sits an admin/clinical portal where senior nurses run a guided MFRS workflow that auto-generates the mitigation flags from a clinical rule library.
