We would like to improve the form to make it like the form on this page: [login to view URL]
As you can see the main improvement is that we need to break up the address field to Address 1, Address 2, City, State , ZIP code.
we also need to break up the full name into "first name" and "last name".
Also when the form is submitted, the message will be sent to the admin [login to view URL] message should be displayed in a form as the attached email.
