I have several patient demographic and background forms for a medical private practice. Instead of having clients print out the PDF, hand write the answers, and deliver them to me I want to:
1. Allow the client to complete the forms on a keyboard or tablet, with the text in the response boxes being easy for them to see and review (i.e. not having anything that they write in their responses disappear from view).
2. Have this file be easily encrypted so the client feels secure in sending it back to me via email (although my email is HIPAA compliant, it is not encrypted, thus I need to protect the incoming files).
3. Have some way to easily and automatically import the completed responses into an Excel file; and if possible, a Word Template
4. The end result is that I can quickly and easily transfer the client's background information into various reports that I write that require this information.
5. I am sure I could figure this out, but hate to spend the hours messing around with Adobe to learn myself.
