We need to create a .pdf survey / report document to send to customers to collect data.
The document should include fields for the customer to enter their name, and their email.
We prefer to have a copy of the uncomplied document in Word format if possible, so that we can modify section titles and questions as necessary.
The document should have 10 sections. Each section will have 10 short questions for the customer where text or a numeric response is required.
The customer should then be able to save the document, (which will save the data entered into the .pdf document), so that they may email the completed report to us.
