Design a PDF form/questionnaire

Create a simple, one page PDF form. It has about 10 questions.
- my users won't necessarily have a professional version of Adobe. Let's assume they have the normal reader.
- some free-field answers
- some drop-down list answers. I need to be able to modify the suggested lists myself autonomously in the future.
- a button at the end submits the form to my email address. This is the parts I don't know how to do, the rest is somewhat manageable.
