Build me an approvals workflow using Google Forms

I need an add-on that integrates with Google Forms to make approval workflows possible. Here's what is required:

1. User submits a Google Form

2. The form summary is automatically sent by email to Manager1 for approval

3. Manager1 can approve, reject, or modify and approve

4. After approval by Manager1, an email summary of the revised form is sent to the next higher manager (if any) for additional levels of approval (if any).

5. After final approval, email notifications are sent to the requestor(s), recipient(s), and manager(s), based on the configuration of the workflow.

The add-on should have a control panel where a user with admin rights can configure new workflows. All form submissions on all workflows should be logged in a Google Sheet with timestamps.

There already exists a third party add-on that does this, but it is paid. I want to develop this so that my organisation can use it without being limited by the number of submissions per month. To demonstrate that you have read and understood this brief, please include in your bid the link to the website of the third party add-on that I just mentioned.
