creation of dvd-label (case+dvd)

Need someone to create a label (cover) for a standard dvd-case and dvd, prepared for printing. The product is a small program for calculating employees' holiday entitlement. It's one of the series, so the dvd label should be created that way to be easily usable as a template for other similar products. On the front page of the case there should be also a nice full-color line drawing with a relaxed man on the beach (under umbrella) showing a piece of paper in one hand and calculator in the other. Will provide you detailed description and text as the company logo as well. Max budget available $40.
