507704 Smartphone + Tab application

Hello all,

I want to create an app for surveys which supports next devices:

1. APPLE: iPhone, iPad

2. GOOGLE: Samsung Galaxy (phone + Tablet), LG Optimus1

The app will consist of next pages:

1. Login page \ enter link

2. (for registered users) Personal account management

3. Welcome page (welcome to survey)

4. Survey (questions, ~10 question types)

5. Goodbye page (end of survey)

The app needs to have a clean look, but sophisticated and reflecting innovation and fun.

I will accept bids only by designers with experience in app design for iOS and Android. I DO NOT NEED any development, which means the graphical layers and objects will be sent to my developer, ready for deployment.
