Build me a custom search form for Oxygen Builder

1. Search has to have dropdown where user chooses which custom post types (not categories) he's searching

2. Attached is a screenshot how it should look

3. I should be able to style all elements with CSS

So you would need to write me code which I will put in a code block element of Oxygen.
