Attached Fairmatic Supplemental application [Please Complete & Return] Attached Apollo Rideshare Supplemental Application [Please Complete & Return] Please send any current marketing or pitch decks including resumes of all management/owners. Big piece here is showing your experience and describing to underwriting how/why your model will be successful. Usage Projections and Geography: What do the next 12 months look like? Please detail expected total mileage for the operation, by state Please detail expected average mileage per trip, by state Please do your best to separate out expected mileage with a passenger in the vehicle vs. expected total (including passenger delivery time) mileage traveled while on your platform, by state Please detail expected # of drivers, by state What is your expected revenue? Do you have any proforma’s you can share? Telematics: What telematic service or software will be implemented within 1099 vehicles to track and monitor fleet/driving behavior? Fairmatic Insurance comes with the vertically integrated telematics which you can review (fourth attachment). The below are for other carriers should we decide to go in another direction. Please include relevant screenshots of tracking and/or dashboards that have access to monitor fleet. Need to understand full tracking capabilities here so please include anything that is pertinent to that discussion. If you need some recommended partners here, look no further than the list below (links embedded): i. Hub Drive Safe App (third attachment) ii. Samsara iii. Arity iv. Geotab Please send over current Driver Terms of Service (I.e., 1099 Driver Contract) and customer terms of service. Please send link to both your driver and customer facing application within the app store. Anything you have currently will be sufficient for now. MVR Reports and Background Checks: What vendor is currently performing background checks? What are the minimum requirements you are expecting from your 1099 drivers? Please walk me through entire driver onboarding process. Most of my customers use any of the following brands (links embedded): i. Lexis Nexis Risk View/Fraud Point/Emailage Services ii. Checkr. iii. Yardstik is another popular choice. iv. Verisk The target completion date for this project is "ASAP." The freelancer is expected to prepare and complete all required documents for underwriting.
