Javascript Font/Color Preview

We are looking for someone to help with javascript on our website. We have 20 fonts and 20 colors our customers can pick from.
What we would like is the viewer to be able to choose their font and color preference, type it into a box and then preview it before placing an order. We currently offer checkout through PayPal premier. If this information could then be transferred to our PayPal shopping cart with a "Buy Now" button that would be ideal. If not, we would like a submit button that will submit the information entered electronically to us and we can provide a price quote.
Basically we are looking for something similar to this:

We can provide the fonts and/or images of the fonts. We'll provide you with the colors.
