Configure Google Analitics tracking code on javascript form triggers

We have a wordpress landingpage with a two steps javascript form embebed and want to configure Goals/actions to measure people actions allong their iteractions:

1: Users that have submitted step 1 (clicked button move ahead)

2: Form submission (clicked buton submit)

3: After submission additional request (cliked button additional request)

Google Analytics account

Landingpage built on wordpress using Avada Theme

Javascript custom form

We'll send link to landing page and require that code is sent to us, with instructions on where and how to insert it. If additional information is needed we'll supply it.
