Add an SQL table with a trigger

We have an MIS system where we add jobs with a due date. I want a table created that maintains the original due date, and another column which updates a revised due date. I also want a counter that will track the number of times the due date was changed.
The flow would work something like this...
New job entered and saved-----> Triggers a new record to the due date table --->

[login to view URL]
Due date modified-------> Triggers an update to the revisedduedate and changes the duedatecount to +1, +2, etc...
We go in and out of these job records all the time, so I'm guessing you may need to incorporate some logic so it's not erroneously counting revised due dates everytime someone saves a change.
I think this is a fairly straight forward tweak that can be handled with a stored procedure.
