Create an Expo-based prototype of a simple healthcare app using React Native

I need a React Native Expo-based prototype of a healthcare card (app) "Schedule a visit" developed for me for this contest: [login to view URL] I don't have the designs, I only have a specification of what I want to see in that card (almost a scenario of a user behavior). Also there are basic technical requirements of the contest, so the candidate will have to register for the contest at the nickname I specify in order to obtain an access to the contest's Github repo with the source code of a sample app and technical guidelines. Deadline of the contest is August 6th, 2018, so I need the prototype's Expo link and its code pushed to the contest's Github repo by August 4-5. My specifications will be sent to the candidates directly. In total I would like to receive 3 'Schedule a visit' prototypes from 3 different developers based on my 3 separate scenarios.
