I have a new install of osticket that I would like to have the fields customized. The system is setup to be a tech support issue tracking system.
I am going to apply the system to be used for a school to track referrals of students to the principal foe discipline tracking.
The form is on this page and all fields need to go through to db and email that goes to admin or staff.

Here are the fields:
Date and time: This is automatically recorded I believe
Student Name: text field (required)
Grade Level: drop-down K,1,2,3,4,5,6,7,8,9,10,11,12 as options
Teacher: text field (required)
Teacher Email: text field (required)
Teacher Phone: text field
Actions by Teacher: Drop down with multiple select (Warning, Conference with Child, Conference with Parent, Other)
Description of Infraction: This should be very large text area
Help Topic will not be on the form it will be fixed as Discipline Referral
Subject will not be on the form, but it will be the Student's Name
Priority will not be on the form, all will be set to default
From will be the Teacher's name
