77192 order form project

Hello- I am building a website that will have 2 order forms. I need both of the order forms to submit the information into a secure MYSQL datase (2 maybe). My client needs to be able to log into the database(s) and retrieve the orders so that she can print out the information and process the orders. She also needs to recieve an email after every order so that she knows to go check the order form. In addition, I wanted to explore other ways in which the php can help me to generate a dynamic form which displays certain information if one field is checked and different info if another field is checked. I am not sure to what extent this is needed, but we are looking for ways to make the form more user friendly and less confusing. More information to come.
