PHP or HTML5 (or other web) script to add a character (#) in front of each word entered in a text box

I am looking for a script created (uploaded to a web page) that takes text set by the user and then outputs the same text with a character (#) before each new word. This needs to work on desktop and mobile devices if a user navigates to the URL. I have added an image of the mock concept of this.
The user flow is as follows:
1. User navigates to [url removed, login to view] on desktop or mobile device
2. User sees "Enter text" text field (need some way to limit how much text they can input) and they type in text such as "NY Giants Superbowl Champs" (without quotation marks)
3. User hits some sort of button or "Enter" on keyboard key
4. User sees text outputted to the right of the text field (from #2) with the following "#NY #Giants #Superbowl #Champs" (without quotation marks)
Optional like to haves:
- ability to copy the text in the output field (#4 above) with a button
- ability to post the outputted text (from #4 above) to Facebook, Twitter, or Google+ (requires they're logged into those services somehow)
- ability to log what people are entering (from #2 above, but after they've submitted #3) and paste as a stream on a separate page like [url removed, login to view] (I can provide mySQL DB)
