Secure Forms

This project requires making secure forms for a site being developed by us. This is a re-design of a current website. We are looking for someone who can take these current forms, improve them by being creative and implement them into the new design. There are about 4 forms. Once the user submits the form, it will be sent to the company via e-mail. If possible, we need a nice output (maybe html) so the look is professional. Looking for a professional person/company who can handle this and finish this in a timely manner. Payment terms as follows: [login to view URL] recommends not paying until the project has been completed. First 30% after the initial testing and approval of the site. Remaining 70% within a week of operation to remove any bugs, web site quality issues or performance issue. Some questions for the bidder's company: Does your company bid include recommendations and suggestions for site improvement? What number of days will you require to complete the project? When will work begin once acceptance has been provided? What is your policy if your firm goes past the project deadline? Would you agree to sign a Non Disclosure Agreement?
