Looking for a custom Outlook 2000 form to collect information and allow exporting to Access. The form is a timesheet which will offer the recipient the ability to select whether to enter their time for the past week in either hours or percentages, across 5 categories. There will be a pull down menu for each category giving more detail on what is included in that category. Ideally, this will use the functionality of the meeting request in Outlook, to allow reporting on who has responded and who has not. This will be done as a "one-off" form, with the code included in the email, so the forms don't have to be loaded on the user's PC.
