I have the Form Maker plugin for Wordpress that I'm using for a client, this plugin works for my client because it creates forms and saves the submissions on the database so they can be also seen and edit/filter form submissions on the backend of the Wordpress admin, which the client wishes to be able to do.
The plugin also has the ability to connect to another mysql database and send the submission data (SQL Mapping), but only mysql.
The client wishes for each form submission to also be sent to a SQL Server database where the data will also be stored, the other way around also, the data can be changed on the SQL Server and sent to the mysql database using the plugin.
They wish to do this using an ODBC connection with stored procedures. I have the form maker plugin files (premium plugin) and have made some changes to the plugin to also incorporate audio recording which is already working. You can check out the form that is going to be used here:

Thats what is needed basically, I have ftp access to hosting and phpmyadmin to the mysql database, the client is going to start creating their odbc and SP queries today/tomorrow. This should be up and running at the most at the end of next week so only bid if you are very experienced with mysql, SQL Server and PHP/Wordpress and are sure you can do this quickly.
