Award Show Image submission and management database

I'm starting an advertising award show. I need the following ability: 1) Users submit filling out a form. Each submission has a different category, and they can either enter a single piece for 1 price, a 3 piece campaign for another price. 2) I then need the form to take them to Paypal so they can pay me! 3) From there, they need a login to the system - they will need the ability to upload PDF, JPG as well as quicktime video. I would like the ability to limit the file sizes of these submissions. 4) Once they upload the files they cannot upload anymore unless they fill out another submission form and pay again! 5) I need a 2-level administration panel to browse all the images and movies. the Admin level will have the ability to hide or unhide entries. The Judges level will have a rating system where they only see images/movies that are not hidden and can rate them on a scale of 1 to 10. They can only rate each image one time. Admin has the ability to create judge accounts. 6) Ability to set Gold, Silver and Bronze thresholds (ie: anything above an 8.5 wins gold, 7.5 silver, 6.0 Bronze) for winners and export winners to XHTML for display on the site. I can build the basic scheme for the form - it just needs to be coded to work. Server is on a typical C-Panel PHP SQL/Linux setup.
