All the validation of the form fields have been done on the front end. Right now, everything works locally. The first thing I'd need is to make sure it works on a hosted environment... I've had some bugs on my server that I haven't had when testing it on my local machine.
After that, I need to write the process-form PHP that occurs when the user hits "submit". This has 2 parts:
1. Kick off an email that contains ALL the fields to a specific address in a readable format. They don't need to be perfectly formatted, but if you could actually copy the "summary" screen at the end of the form into an email, that would be perfect. (the email would include the page totals, and final total).
2. Save the field values to a .csv file or tab delimited file (anything that can be easily opened in Excel) where each new entry gets added as a new row. This .csv file will need to stay on the server and include all the fields EXCEPT the CC info.
The CC info is only sent by email, not stored on the server. I know email isn't much more secure than a server, but that's what the client wanted and I warned them of the risks.
Files are all available (zipped) at [url removed, login to view]
Ignore all the html files except for "[url removed, login to view]". That is the form I'm trying to get working.
Your haste is absolutely appreciated.
