I need a simple PHP form processor.
There are 12 fields coming in via a one page HTML form.
The HTML is done for the form.
The PHP form needs to perform the following functions:
-PING an API. Basically, it will pass the HTML variables in the following format (with dummy data inserted for your clarity) to [url removed, login to view]+E.&field2=Coyote&field3=123+Fake+Street&field4=&field5=Help&field6=NE&field7=27613&field8=&field9=9199199111&field10=9199199999&field11=100&field11=3
(With field# substituted for the field name, of course... GAF just wouldn't let me post the field names.)
This API will pass back a SUCCESS or FAILURE message and an ID. This could be ignored or displayed, whichever is easier for you. The user also needs to see a Thank you for your submission message. This could be in a popup window, a separate window, whatever. To be honest, I don't even care if the user ever sees the page with the API results, so long as the API WRITE is actually done (as listed in the format above) and they are taken to a "Thank You" page.
I will let you know the exact address of the API processor page via PMB. This should be no more complex then "pinging" that API in the above format and sending the user to an HTML page. I don't care how the API results are handled. If you can show me the results, Great, if not, that's fine too, so long as the core functionality I want evolves:

A Thank you page is displayed
You do not need to format this thank you page in any way. In fact, you can just set it to [url removed, login to view] or something... I can make the HTML page myself. All I need is the PHP processor.
