509958 Online Questionnaire/Survery Form

I need to create a form for a survey or questionnaire. Essentially a user will read a question and select one of two answers. Depending on what they select it will load a new question. There are only about 10 questions. The questions need to reload automatically in the box and not reload the whole site.
