PHP invoice page

Job Description:
This project calls for an invoice page that our field technicians will use to issue invoices for FRU's. It will be served from a private section of our website. The invoice page must include our logo and we must be able to change the image. Our current logo is in the attached zip archive. The form will have several calculated fields, such as tax and quantities. The form must save each invoice in a MS Access db. If you can't do MS access do not bid on this project. We would prefer PHP to handle the form processing. We also require another form that will simply allow us to recall a past invoice. The invoice should print cleanly from the web. No header or footer. Please feel free to design a basic layout and show us, this will influence our choice.
