565692 Two Custom Forms

Hi, I need someone to help me create 2 relatively simple forms. Each form has about 10 fields; one is a contact form and the other one is for cake tasting appointments. - The forms will have some required fields. - The forms need to load on the same page with a "Thank you!" message. - The forms will have text, text area, upload, drop down menus, and possibly a calendar date choice + radio button combination. - A check box that adds the user name + email to a database (MySQL) and then a way to get the database into an Excel spreadsheet. I expect the project to be done within 2 days and an extra 2 - 3 for errors and changes. NOTE: I am looking for someone who I can contact for more projects in the future.
