Joomla 1.5 Plugin

I am using Joomla 1.59, and want a 1.5 native module/extension/plugin that duplicates the functionality and layout of the pages below. I do not have access to the backend, but you can see the data that is being collected and displayed. If you are familiar with LQM, you can use that to develop if you like. 1.
