Small calculator projects

We have a number of small calculator projects that we are launching. First project is a BMI calculator tool, that has to be published via Wordpress.
Some requirements:
- Use a form that makes an AJAX call to a PHP file.
- Use jQuery for triggering.
- Excellent English communication skills
- Show relevant examples of your work
We have screenshots and example websites available for qualified freelancers. One screenshot is attached.
After a successful project, more projects will be awarded.
