I need a PHP page scripted that takes form input from a user. The design of the page doesn't matter; I will move your code into my own page design. I require someone with php/mysql/jquery knowledge as I don't want the page to refresh as queries to the database occur.
Data to be captured in the form:
-Incident Number (textbox)
-Short Description (Text area)
-Start Date and Time (textbox - but on click it pops up a jquery datepicker and timepicker [Black and grey theme])
-End Date and Time (textbox - but on click it pops up a jquery datepicker and timepicker [Black and grey theme])
-Priority (Drop down list... I will add my own values but for now one option can be 1 and the other 2)
-Link to Investigation (drop down list... options queried from a mysql table displayed below)
-Link to CI (textbox with autcomplete and an add button)

Values in the "Link to investigation" drop down list are read from a mysql database called "Problem Management". The connection string to connect to this database already exists and can be included in the script that you code for me by include('[url removed, login to view]'). The table in the database is called "problem". Among other fields in this table, the three fields that need to be queried are
*problem_id (Primary Key)
So the HTML code for the drop down field will need to be

*** "Link to CI" - jquery autocomplete text box ***
Using jquery's autocomplete text box functionality I would like to type in certain values into a text box and have suggestions made based on my input. The suggestions should come from another mysql table which i will detail in a moment. When the user selects one of the suggestions they can click the add button. Each time they click the add button, their selection should appear below the autocomplete button with button beside it saying remove. Users can continue to add/remove entries but each time they add or remove, their selections are not yet committed to a database (which I will detail in a moment). No Refreshing please, all to be done with jquery with all background processing. Users can only pick suggestions; if they type something that is not a suggestion - it is not to be committed back to the database from which the suggestions come from.

*** Submitting the form ***
When the user submits the form; four things need to occur
1 - A table called "incident" needs to be inserted with:
*incident_id (int)(pk) <-- This is autoincrement so no need to specify
*incident_number (int) <-- From the Incident Number textbox above
*incident_short (text) <-- From the Short Description text area above
*incident_start (datetime) <-- From Start Date and time above
*incident_end (datetime) <-- From End Date and time above
*incident_priority (int) <-- From Priority drop down list above)
2 - The table called rel_incident_problem needs to be updated with a new record:
*rel_incident_problem_id (int)(pk) <-- This is autoincrement so no need to specify
*incident_id (int) <-- From the incident_id generated by the previous query where incident details were inserted into the incident table
*problem_id (int) <-- From the problem_id that was active in the drop down box
3 - The table called rel_incident_ci needs to be inserted with a new record for each CI that was added via the autocomplete text box
*rel_incident_ci_id (int)(pk) <-- This is autoincrement so no need to spcecify
*incident_id (int) <-- From the incident_id generated by the query where incident details were inserted into the incident table
*ci_id (int) <-- From the CI that was added
4 - After successful submission there should be a message on the top of the page stating that the query was successful or was not successful. The form should be blanked so that they can insert another record.
