PHP Based Website Backend Admin MySQL Database Interfaces

Create Admin Interfaces to existing MySQL tables for PHP based website.
One interface will be a record from the main table, along records from another related table. (One to Many.) This interface will allow adding/editing/deleting main records as well as secondary records. It will require input validation as well as checking for duplicate records.
The second interface will be to view a log table, allowing filtering and sorting on any field.
All code must be human readable, fully commented and not encrypted.
Should work on all major browsers in a 1024x768 resolution (fluid resolution handling OK)
Code will become our property.
Must work on PHP 5.2.5 and MySQL 5.0.90.
Code must be able to be placed in any folder to function (no hard-coded path restrictions.)
Sample Database Tables will be provided upon selection of freelancer.
Would prefer that the database tables not be modified (names, field types,etc,) but will consider it if needed.
To clarify, must work on PHP 5.2.5+ as well as MySQL 5.0.90+ (up to currently released versions.)
Fluid resolution handling would be preferred.
All major browsers include (but not limited to) IE8+, FF3.6+, Chrome 5+, Safari 5+, and Opera 10.6+.
Would be a + to work on iOS Safari, and webOS browser, but not required.
Cannot require a browser plug in (such as FLASH or Silverlight.)
The application does not require a log in or user (that will be handled externally.
Should work with the StyleVantage style by StyleShout. Does not need to conform with the 3 col format, just allow the header/footer and be compatible with the CSS of that template.
