Scheduler web application simple design but custom needs

ONLY MYSQL, PHP AND THOSE WHO HAVE DEVELOPED SMALL APPLICATIONS APPLY. I have worked for a few years with a medical company that has used a Joomla component for an appointment scheduler and also a Joomla component event manager. Both have created a mess each year. The needs are so simple but they are custom and each of the prior applications has too many modules so when you customize it messes other things up. I need a custom database software built. You must be an expert in database design on MYSQL and small website application creation. Their will be a front end and a backend. The backend must have security features built in. Backend: Will need a simple process to enter the daily schedule of appointments available. They create custom time slots with a Max # of attendees for each day. Administrators will need to be able to quickly view and search appointments by; Last Name, Appoint Date, or other criteria they specify. Administrators will need to be allowed to change an individuals appointment or cancel that appointment from the backend. Administrators should be able to print reports of appointments booked with all required fields for a day, Month or custom time frame. The list of daily appointments should shoe administrators at a glance if all of the time slots for that day are full. Frontend: Must be designed simple with large buttons which clearly define the next step. Home screen = Make an appointment and cancel an appointment. No Login to make an appointment, instead they would select the by calendar view or by list view the day they want their appointment, if the day is full or no appointments are entered for that day the day will be grayed out . That day will bring up the list of time slots available for that day and it will show how many openings are still available for that time slot. If that time slot is full it will be grayed out. Next they will fill in less then 6 fields of required information and they will complete. Completion send them a confirmation email with instructions on how to cancel the appointment. Individual will be able to see all his appointments by entering certain fields from the registration. A list of his appointments would pop up with a cancel button. Confirmation message and email follows. A reminder is set to send out to each email 2 days prior to their appointment. This is the most of it with mild variation as we get into it and need to resolve design or glitches. I must have this completed in 2 weeks from start. No exceptions.
