SQL Data Entry System - Web GUI with some validation required

I am looking for a simple data capturing system (using sql) with a web GUI. It should include a data entry form with: South African ID Number validation (Date of birth valid and field auto complete, male female title verification etc, length check) South African bank account number/card number validation (basic length and bin number checks - These rules I don't have) South African address check - Only suburb and post code selection (List is available on the SA post office website) A few other standard fields - 40 in total including above Export date range into .csv (field structure can be provided) User management (5 users - 2 levels) Ability to add and remove more fields to 3 or 4 drop down fields in the form (Sales agent, campaign etc) Reporting: list of records for a specific date range per user, campaign and sales person + export to excel. Background info: A basic access system is in place but the data validation is being done one-by-one by the capturing agent and is taking forever The csv file its exporting is sent to a client to be used in their system but currently needs to be reformated slightly and gets rejected from time to time as a capturer may make a mistake in one of the crucial fields like the the ID number and the entire file gets rejected with no description and takes a while to debug
