In short, I have a series of surveys that I use built with HTML/PHP/MySQL. I'm now at a point where I need to authenticate some users. I know I could simply download an option, but I want some specific details. I added a JPG with some details.
I already have most pages built, like the index.php=login page, along with the survey pages. I need help developing the authentication system.
1. Each user of the a particular survey gets a 6 digit code. The code is unqiue to each user, so the system must authenticate the user's code to one stored in the database.
2. Each survey has a unique id, so the authetication needs to check if this user has already filled in the survey.
The idea is that each user can only fill in a particular survey once, ok, not too complicated.
What I want to be able to do is add this same code to the top of each of my PHP built surveys. Each survey will have a $surveyid that will make it unique.
I will supply to the winner a login file and an example survey.
I'm open to suggestions to make this all work.

there is no password, so just hit submit to see an example survey.
One other thing I need help with...notice the checkboxes, if you check the box a textbox appears. However if the pages reloads, for example if required information was left blank, those textboxes disappear. I need to resolve this issue as well.
