Secure & Image MySql Upload

I am looking for a secure upload and an image upload addition for my website. The Secure Upload will include a 'random' word or numbers that is shown on the screen and must be typed in for the upload to happen, it is usually broadcast in a wavy, computer-unreadable format. The Image upload will be to upload images into my MySql database. I'm not sure the code myself. I don't think it's too difficult. Must be code that allows for Jpeg, tiff, and other common picture formats. the code must be secure and protect against site control via the upload. and allow up to three pictures and limit the size of each image file to below 10 megs. Thanks
