Online Survey Project

I need a survey created that consists of four matrixes. Please see the attached Excel spreadsheet while reading the remainder of this description, it will make more sense. The survey will be posted online. I would prefer it made using PHP/javascript (and MySQL if needed). The survey looks like a spreadsheet, there are answers to be inserted into the cells. Answers will can be inserted into the yellow cells. Answers are not required in each cell, they can be left blank. Totals that appear for rows and colums (right and bottom) are for the convenience of the person filling out the survey. They do not need to be saved. Grid 1: The green cells at the right and bottom will total the rows and columns. The total of the entire grid cannot exceed 100%. If the total is more than 100%, the survey should generate an error until the answers are equal to or less than 100%. Grid 2: This grid will have drop down menus with three selections. (High, Medium, Low). There are several squares where there will be no choices, they are indicated by N/A. Grid 3: Similar to Grid 1, except only rows need to be totaled, not columns. Again, the total of the entire grid cannot exceed 100%. Grid 4: Similar to Grid 3. The survey should require a password for each user. The passwords should be read from a file that I upload to the server. Each person will have a unique password. This password will help us link the survey to another related survey we are developing on another platform. Finally, the survey should save a data file with all the responses for each record. I supposed each answer can be assigned a grid number, sort of like a spreadsheet, so that it would be easy to generate a report from the data file. I would like to be able to export the data file using a button or have the data written to a file that can be downloaded via FTP. I need someone who can start on this IMMEDIATELY. My client got this to me VERY LATE, and we need to start testing it within 48 hours for deployment within 5 days. Please do not bid if you are not able to devote immediate attention to this project. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform Web based application, running on Linux, Apache, PHP, etc.
