490546 PHP Scripting for Web Faxing to Gateway 2

I have an API sample showing how you can post your Fax to Fax server for delivery.

However, the API is not ready to start sending fax immediately and the HMTL part of it for user interface is not included, I want an expansion of the php file to be ready to run on browser with user interface.

What we need is the feature to send fax from a single webpage, which will contain 1 or more numbers, and 1 or more attachments
