You will need to implement an online assessment tool for 2 primary types of users. Assessment takers will be completing a survey comprised of 10 questions with 10 answers presented in a table format. Facilitators will be creating the assessment instances, sharing links with potential assessment takers or conducting surveys on their own device when meeting with them face-to-face. An administrator role will be added later to manage the contents of the survey and who has access to it. See Functional [login to view URL] for details.
The functionality is intended to be delivered in several releases:
1. Assessment creation, completion and follow up.
2. Sharing of assessment results and comparison with a baseline.
3. Management of authorized facilitators.
4. Management of surveys.
A graphic design will be provided. It will need to be implemented so that desktop, laptop and tablet users can use the online tool. Development, staging and production regions will be set up with PHP 5, MySQL 5 or PostgreSQL. Although not mandatory, the implementation can be done in WordPress if that helps. The first release must be in production by June 1, 2013. It should support a pre-defined survey and 10 facilitators until the functionality to manage them is completed in later releases. Iterations, user stories, backlogs and other agile project management techniques are expected to be used for all releases.
When bidding, please provide an effort estimate to complete release 1 only, since requirements for later releases may change.
