464760 Question Quiz Needed For Website 2

I have a 6 question quiz on a website. Each question has 2 to 5 optional answers to choose from by checking a radio button. Each question can only allow 1 answer.

There is a "Get Results Now" button at the bottom of the quiz that is NOT active (can't click it) until ALL questions have been answered. Then the "Get Results Now" button becomes active and they can click it to go to the next page.

However, it isn't functioning properly. Now if someone changes an answer after they've checked a radio button under all 6 questions, the "Get Results Button" is deactivated again.

I need someone who can make the quick change so that this functions properly, even if someone changes an answer.

In addition, I'm looking to have a progress bar graphic on the next page that looks like answers are being analyzed (it doesn't actually have to calculate anything... I only want it to appear like something is happening in the background). Then after about 5 or 7 seconds I need an email form to appear that requests their email address to get their results.
