Our customer site uses X-cart. Apparently there is a problem with permissions in the application that allowed a hacker to place text and a link within pages that use X-cart. We need to troubleshoot the problem and make sure that the file permission system that X-cart uses will not let that happen again.
Below is the explanation from our host:

restricting file and directory permissions to the minimum necessary for operations will miminise their impact. Many common scripts, X-Cart not excluded, are fairly liberal with their permissions and leave much of the site's content vulnerable by default in the name of convenience."
