I have a questionnaire that'd I'd like to have put online.
The website is built on WordPress, but the questionnaire can be accessed from an outside (non WordPress) domain if it has to be. (I haven't been able to find a developer who can make it work in WordPress.)
I am looking for a developer who can create a form (or utilize the existing form that has already been created, but was not completed) and set it up to deposit information into a database. The information needs to be saved in real-time, field by field (since the questions & answers can be lengthy).
It needs to have the following functionality: to allow a user to create a username and password, and the ability to return to the form multiple times (and edit responses) before final submission.
The admin should be able to login to Wordpress or to the backend of the database to view/export the data submitted, as well as the option to notify the admin that a form has been submitted. All the users who have filled the form will be listed out with an Export button next to their names so the admin can export the data for any user one at a time. For the export function, the data should export into Word and Excel with both options provided.
The form would be divided into approximately 30 pages with each page carrying a different section. There will be a "Save as Draft" and "Next" option at the end of every page. Users will be able to save their form at any stage and logout. Once they re-login their last filled details will appear in the form. All fields to be filled/validated. If any field is left blank the user will need to fill it up before proceeding to the Next page.
If they click on Next, they will be taken to the next page and so on till they reach the final page where there will be a Submit button. On submit, the form will be emailed to the site admin's email address as a Word document.
If you can get Wordpress integration to work, the form will be a part of Wordpress pages and be in a SSL area. The header, footer and the page indentation will remain as in Wordpress. The form will be accessible after users login by entering their password as is on the site now and all the sessions will be tracked so if a user navigates to some other page while filling the form and then returns to the form the data will still be there. Users will need to click on Save as Draft in order for information to be saved before they logout. Even if a section is incomplete and Save button is clicked, the data will be saved.
