Re-Build Web form that uploades submitted data to existing db

Job Description:
We require the Re-Building of a single page Web form that uploads submitted data to an existing db.
Some Fields need to be repeatable for the end-user.
This is on a WP site built with Divi Builder.
Would require the form be built with Advanced Custom Fields PRO ([login to view URL]) & ACF Custom Database Tables ([login to view URL])
The existing db needs to be preserved as we have another web app we use to manage and implement changes.
Site is hosted on a hosted platform that uses cpanel.
