Basic Zendesk UI for customers, PHP, Zendesk APIs

We have a basic Zendesk account (support only). Zendesk doesn't offer a user interface to our customers for our current plan. We want to have an in-house interface that uses Zendesk APIs, written in PHP.
We need these features:
- require customer to authenticate
- list customer's open tickets
- list customer's closed tickets in another page
- form to add a reply to a ticket
- form to create a new ticket
- very simple but professional layout, with out logo
