build 2 pages with dynamic table with dynamic multi select dropdown using php and jquery

I need 3 pages Search - displays content in a 2 column table create/Display/Edit - Shows the selected row details in a table (if create shows empty, if display shows the search results in ready only mode, edit shows the search results in edit mode) Dataelem - Displays Create/Display/Edit page table one of the column values data as table header Search page fields Edit field with search button Create/Display/Edit page Text field, Text area field, Multi select dropdown with checkboxes, A table with 4 columns. Each column has one dynamic dropdown, 4th column has a dynamic dropdown with multi select option with check boxes. 2nd column drop down values are pulled from database based on first drop down selection. Same applied for 3rd and 4th column. should be able to add and delete a row. By clicking submit should provide the all selected drop down values and their value property into a json format. If the page is navigated from Search page then displays in ready only mode with an edit button By clicking edit button except text field every this enabled for changes Dataelem - Displays Create/Display/Edit page data elements as table header Display one of the column selected values as table header and have add row option to enter values in each column By clicking save button will share the info into database. .
