Build simple form page in Admin LTE template

I need a form built in Admin LTE template with the same fields as a certain website. The website has a step by step wizard, but I want to add the fields in one form, one step. Next to each form field should be a question mark icon that will open a modal popup that will have text in it. This is for help and guidance. Easy to add as Admin LTE has this built in. It should not take you more than 3-5 hours to build the form.
The form should use the style from Admin LTE and should be able to save the values to a MySQL DB. This should not take you more then 1-2 hours.
After saving we show the values in a document where they will be inserted into predefined content to create a document. This should not take you more than 2-3 hours.
Multiple users should be able to fill out the form and access their data via email and password. They should not see the data of other users. This should not take you more than 1-2 hours. So in total this should be done in about 10-12 hours. Upload page and create MySQL via Cpanel for test.
