The form will take the users input and concatenate the user's input into a web address and go to that website after they click search. Sort of like google but for a specific site. Example they search "dog bowl" and click search you would concatenate their search term to where it says CONCATENATE SEARCH WORDS HERE in the first url

As you see each word has a plus sign between in as seen in the second example dog+bowl
Once they hit search it will take them to url where you added the search text into it.
