Change Video upload from Flash to HTML 5

We have a website which allows users and Admin to upload video files using Flash.
The user has a Video upload button that allows for the Video file to be selected then uploaded to the third party site using the API. This is similar function on the User side and also the Admin side, but the pages are set up differently.
It was working fine till a month ago, but for some reason, this is not currently working properly so we need to fix the issue and move to HTML 5 as soon as possible.
The site is based on YII framework and uses PHP, Javascript and HTML and the database is MYSQL.
We are looking for someone who has prior experience in the following:
1) YII Framework
2) Video upload
3) API integration
4) PHP, Javascript and MY SQL
5) Flash to HTML 5
I have attached the screenshots of the Admin side and User side for better understanding of the work to be done.
